Who is eligible? Full-Time Employees (Average 32+ hours per week)
How does it work? Employees pay 100% of the health insurance costs, Assisted Independence reimburses 50% of employee cost per month on check.
What does this cover? Your policy may have dependents on it. Assisted Independence only covers 50% of the cost for you as the employee. It is your responsibility to cover your other 50% and the full cost of any dependents. Vision and dental are not covered.
What do I need to do to get set-up? Request a quote by emailing (please CC the Director of Recreational Therapy) your full name, address, birthday, and expected annual income to Janice Hildabridlle at jhildabridle@onedigital.com
