Once the Service Authorization is received, the Recreational Therapist will have access to the Person-Centered Individual Support Plan (PCISP). It is a requirement to read each individual supported PCISP.
In the PCISP, you will find helpful information about the individual pertaining to each life domain. You will also find contact information for all people in the individuals life. It is a requirement to add the contact information for the following people:
Individual
Parent/Guardian (if applicable)
Case Manager
Behavior Consultant (if applicable)
Music Therapist (if applicable)
Vocational Therapist (if applicable)
Direct Support Staff (if applicable)
House Staff (if applicable)
