Basic Information
In this section, you will find the individuals’ name, address, phone number, and email address. You will also find their legal status.
Living Arrangements
The Living Arrangements tab has whether the individual lives in a group home, in the family home, or independently.
Relationships
This section will list all of the people that provide support to the individual on the waiver, as well as any primary or specialized doctors they may see.
Diagnosis
This section will list the individuals’ primary and any secondary diagnosis they have. Some individuals will have a Waiver Diagnosis and a “Mental Health or Medical” Diagnosis.
