For each individual, you will need to upload an Assessment and Treatment Plan when you begin working with them. These documents will be updated and uploaded to the Bureau of Disabilities Services (BDS) online portal annually.
Step 1: Login to the Bureau of Disabilities Services (BDS) online portal.
Step 2: Go to the individual’s personal profile.
Step 3: In the top, select “Documents”.
Step 4: On the top right side of the screen, select + Upload Document.
